MEDICAL
With medical you’ve got three choices. Each plan covers the same illnesses, symptoms, and procedures so the question boils down to…how do you want to pay for your healthcare over the next 12 months?

HSA 4500 PLAN
The HSA 4500 Plan has a higher deductible, but lower premium than the PPO Plans. This plan allows you to participate in a Health Savings Account. Also with this plan, after you meet your deductible, there are no further out-of-pocket costs except for prescriptions.
PPO 3000 PLAN
The PPO 3000 Plan has a lower deductible but high premium cost, which is the amount that is deducted from your pay. This plan has copays for things like doctor visits and lab tests, which you would continue to pay after the deductible has been met.
PPO 1000 PLAN
The PPO 1000 Plan has the lowest deductible but highest premium cost. This plan also has copays, just like the PPO 3000 plan.

HSA - HEALTH SAVINGS ACCOUNT
A Health Savings Account (HSA) combines high deductible health insurance with a tax-favored savings account. Money in the savings account can help pay the deductible. And money left in the savings account earns interest and is yours to keep.
For 2021 you can contribute up to $3,600 if you enroll as a single in the medical plan, or up to $7,200 if you enroll as a family. Additionally, if you are age 55 or older, you are eligible to make a catch-up contribution of $1,000.
These amounts include United Zion's contribution. You may elect to change your contribution amount at any time throughout the year!
United Zion will make a $1,000 contribution if you're enrolled as a single, and a $2,000 contribution if you're enrolled with dependents!
Team members eligible for benefits as of April 1st will receive an annual contribution of $750.
Team members eligible for benefits as of July 1st will receive an annual contribution of $500.
Team members eligible for benefits as of October 1st will receive an annual contribution of $250.
FSA - FLEXIBLE SPENDING ACCOUNT
A Healthcare Flexible Spending Account (FSA) is a health care account that lets you set aside pretax money to help pay for qualified medical, dental and vision expenses that occur during a 12-month period.

Now let’s talk about the extras. Whether you want to think about insurance to take care of your family,
or to cover some expenses if you're out of work, we’ve got options for you.
LIFE INSURANCE
You have the option of purchasing additional coverage for yourself, spouse and children. You may choose coverage at a minimum of $25,000. This plan is administered by MetLife.
SHORT-TERM DISABILITY
The Short-Term Disability program provides financial protection for you by paying a portion of your income when you are Disabled. The benefit is up to 60% of your weekly earnings with a maximum of $2,000 per week. This plan is administered by MetLife.
LONG-TERM DISABILITY
The Long-Term Disability benefit is 60% of monthly earnings with a maximum of $10,000 per month, depending on the plan you choose. This plan is administered by MetLife.

HOW MUCH IT COSTS
If you enroll in one of our PPO medical plans, there is no per-visit cost for you. If you enroll in our HSA plan, there is a cost of $99/visit for initial sick visits. There are no fees for follow ups of sick visits or preventative care visits. All preventive and wellness visits are of no cost. Click here to learn more.
To help pay for these visits, you may use the $1,000 or $2,000 HSA dollars that we contribute into your HSA bank account.
