THREE HEALTHCARE PLANS
We offer three healthcare plans from which to choose. Take a look at our plans here.
We will continue to offer no-cost care for team members who select either PPO plan. For team members who select the HSA plan, your first sick visit is $99. All preventive and wellness visits are of no cost. Learn more about Rock Medical.
If you're enrolled on the HSA plan, we're giving you free money: $1,000 if you're enrolled as a single, and $2,000 if you're enrolled with dependents. Learn More about HSAs.

We appreciate your dedication and service and are proud to offer a benefits package that is valuable, flexible and competitive, providing right-size coverage. If you are a full-time team member regularly working over thirty (30) hours per week, you are eligible to participate in the Benefits Program. You have the option to enroll yourself and/or your eligible dependents in our benefits program.
Eligible dependents include: (1) Your legal spouse that is not offered coverage through his/her current employer; and (2) Your children up to age 26. Only those dependents who meet the eligibility requirements can enroll for coverage.
With medical you’ve got three choices. Each plan covers the same illnesses, symptoms, and procedures so the question boils down to…how do you want to pay for your healthcare over the next 12 months?

HSA 4500 PLAN
The HSA 4500 Plan has a higher deductible, but lower premium than the PPO Plans. This plan allows you to participate in a Health Savings Account. Also with this plan, after you meet your deductible, there are no further out-of-pocket costs except for prescriptions.
PPO 3000 PLAN
The PPO 3000 Plan has a lower deductible but high premium cost, which is the amount that is deducted from your pay. This plan has copays for things like doctor visits and lab tests, which you would continue to pay after the deductible has been met.
PPO 1000 PLAN
The PPO 1000 Plan has the lowest deductible but highest premium cost. This plan also has copays, just like the PPO 3000 plan.
COMPARE MEDICAL PLANS
With our medical plans you can access care in-network or out-of-network. We recommend accessing in-network care whenever possible, as it will be less expensive. The medical plans are administered by Meritain, using the Aetna Network. Click here to find an in-network medical provider.

HSA - HEALTH SAVINGS ACCOUNT
A Health Savings Account (HSA) combines high deductible health insurance with a tax-favored savings account. Money in the savings account can help pay the deductible. And money left in the savings account earns interest and is yours to keep.
For 2022 you can contribute up to $3,650 if you enroll as a single in the medical plan, or up to $7,300 if you enroll as a family. Additionally, if you are age 55 or older, you are eligible to make a catch-up contribution of $1,000.
These amounts include United Zion's contribution. You may elect to change your contribution amount at any time throughout the year!
United Zion will make a $1,000 contribution if you're enrolled as a single, and a $2,000 contribution if you're enrolled with dependents!
Team members eligible for benefits as of April 1st will receive an annual contribution of $750.
Team members eligible for benefits as of July 1st will receive an annual contribution of $500.
Team members eligible for benefits as of October 1st will receive an annual contribution of $250.
FSA - FLEXIBLE SPENDING ACCOUNT
A Healthcare Flexible Spending Account (FSA) is a health care account that lets you set aside pretax money to help pay for qualified medical, dental and vision expenses that occur during a 12-month period.
Healthcare FSA – Can be used ONLY with the PPO medical plans
Dependent Care FSA – Can be used with any of the medical plans
BENEFITS OF A HEALTHCARE FSA
Use it for office visit copays, Rx, and more
Contribute up to $2,750 per year
Use it or lose it (you may roll over up to $500 per year)
BENEFITS OF A DEPENDENT CARE FSA
Use it for eligible child care expenses
Contribute up to $5,000 per year
Use it or lose it (no rollover)
This optional benefit is available if you want a little extra protection for those pearly whites.
The Dental plans are administered by MetLife.
You will not receive an ID card from MetLife. When you visit a dental provider, let them know our group number which is 5344306 and our plan network is PDP. To locate in-network dentists click here.
Do you or your family wear glasses or contacts? Then this benefit is for you! The Vision plans are administered through MetLife.

Here are some benefits provided to you at no additional cost.
United Zion cares about your well-being and encourages you to use your PTO throughout the year. You can use PTO for things like vacation, personal or family illness, doctor appointments and more. Take a look at our Benefits Highlights for more information about our PTO policy.
Thinking about your future is important and we want to help where we can. We contribute a 50% match when you make contributions in any amount up to 10%.
All eligible team members are immediately, fully vested in personal contributions and become fully vested in any employer matched contributions after three (3) years of service are completed.
Now let’s talk about the extras. Whether you want to think about insurance to take care of your family,
or to cover some expenses if you're out of work, we’ve got options for you.
You have the option of purchasing additional coverage for yourself, spouse and children. You may choose coverage at a minimum of $25,000. This plan is administered by MetLife.
The Short-Term Disability program provides financial protection for you by paying a portion of your income when you are Disabled. The benefit is up to 60% of your weekly earnings with a maximum of $2,000 per week. This plan is administered by MetLife.
The Long-Term Disability benefit is 60% of monthly earnings with a maximum of $10,000 per month, depending on the plan you choose. This plan is administered by MetLife.

With our partnership with Rock Medical, you can receive a variety of services to help you achieve and maintain your best level of health and wellness, including:

HOW MUCH IT COSTS
If you enroll in one of our PPO medical plans, there is no per-visit cost for you. If you enroll in our HSA plan, there is a cost of $49/visit for initial sick visits. There are no fees for follow ups of sick visits or preventative care visits. All preventive and wellness visits are of no cost.
To help pay for these visits, you may use the $1,000 or $2,000 HSA dollars that we contribute into your HSA bank account.
